Request for Records

Decatur City Schools provides every opportunity for every student and specializes in meeting the needs of every child. Special Services encompasses the school district's Special Education Program and the English Learners Program, as well as meeting the needs of students through the 504 Program.

Decatur City Schools' Special Services department maintains special education records of all currently enrolled and former students for 5 years after the student graduates or leaves our system. Special Education Records for graduates or formerly enrolled students of Decatur City Schools may be requested from Special Services. The special education records for graduates or formerly enrolled students are not at the schools.

Students, parents, or legal guardians of students under the age of 18, may request special education records by completing and submitting the "Requests for Records" form provided below.

If you seek general education documents such as a transcript, immunization records, attendance records, birth certificate, grade reports, or disciplinary history, please contact the last school attended in our district.

General education documents for students in Grades 9-12 are housed with the registrar of the last high school of attendance in our district.

How to Request Your High School Transcript: Parchment

Welcome to Parchment. Decatur City Schools is offering this service to make ordering transcripts easier for you.

Please note the following when ordering transcripts:

1. Please order your transcript or other documents several days before you need them. It can take 2-3 days for your order to be processed.

2. Parchment requests are not processed on the weekends or holidays. Keep in mind school is closed for one week for Thanksgiving, two weeks for Christmas, and one week for Spring Break. There are also additional days throughout the year when school is closed. Please plan accordingly.

3. You must request your transcript or other documents using your legal last name when you were in high school. If you use a different last name we will not be able to find your records.
